Family resources

Public invitation. Private family logistics.

Registration information can be public. Dancer assignments, class times, arrival instructions and family records are distributed directly to registered families.

2026–27 registration

Registration for the 2026–27 school-year program is opening. Current families receive registration instructions directly by email. New families may contact Rosas de María or join the program-interest conversation.

For registered families: the complete working schedule—with dancer call times, grade assignments and meeting locations—is distributed directly and is not posted publicly.

One coordinated Rosas family packet

Two required forms. One optional assistance request. Media remains a separate choice.

Start with the intake and participation forms below. Use the financial-assistance application only if needed. The media release is intentionally outside the participation packet so a family may say YES or NO without changing a dancer’s place in the program.

Rosas family packet · Step 1 of 2

Family Intake & Privacy Form

Private operational information: student and guardian contacts, emergency and authorized pickup, health and participation supports, custody or safety instructions, and optional limited school-coordination consent. This Rosas form does not replace or modify any CUSD document.

Complete English pages 1–2 or Spanish pages 3–4. Print, sign and return the completed pages in a sealed envelope. Do not email or upload a completed form.

RDM-INTAKE-002, Version 2.1. Private operational record; not a medical authorization, risk waiver, financial-assistance application or media release.

Rosas family packet · Step 2 of 2

Participation, Medical Authorization & Risk Waiver

Program expectations, emergency medical authorization, ordinary folklórico risks, a limited release, and guardian acknowledgment. This is exclusively a Rosas agreement and does not recreate, replace or interpret a CUSD form, policy or MOU.

RDM-PART-003, Version 2.1. Current Rosas organizational form; not a CUSD form.

Rosas family packet · Optional confidential request

Financial Assistance Application

Use this separate form only when requesting help with program fees, costumes, shoes, accessories, performance or travel costs. Financial-assistance consideration does not depend on media permission.

RDM-AID-001, Version 2.1. Reviewed confidentially and separately from program participation and media permission.

Parent or guardian election

Minor participant media release

This bilingual PDF records a parent or legal guardian's affirmative YES or NO election for photographs, video and incidental audio. English is page 1 and Spanish is page 2; complete one language page only. A NO election does not affect participation and permits only incidental appearance in distant, wide-angle, crowd, group or full-stage images of a public performance when the child is not singled out.

Complete the fillable PDF and type your full legal name in the electronic-signature field, or print and sign it by hand. Email the completed form directly to RosasdeMaria@RandallStone.net or return a printed copy in a sealed envelope to an authorized Rosas de María representative. Do not submit it through a public website upload.

RDM-MEDIA-001, Version 2.1. Media permission is separate from program participation and may be withdrawn prospectively in writing.

Supplemental family expectations · Spanish

Acuerdo de Compromiso Familiar 2026–2027

This Spanish agreement gathers family contact information and confirms attendance, communication, costume, conduct, volunteer, and payment expectations. It supplements the required Rosas forms above; it does not replace medical authorization, privacy, media, financial-assistance, or CUSD documents.

RDM-FAMILY-004, Version 1.0. Return the completed form only through the private channel designated by Rosas de María; do not upload it to a public website.

Separate school-district requirement

The CUSD BLAST form stays CUSD’s form.

Families using the Rosedale BLAST release process must also complete the district form. It is presented separately because it is not part of the Rosas family packet.

Required for Rosedale BLAST students

CUSD BLAST Student & Contact Information

This is the official Chico Unified School District BLAST form required for students participating in BLAST at Rosedale Elementary. Page 1 is in English and page 2 is in Spanish. Complete the form, print it, sign it by hand and return it according to Rosedale/BLAST instructions. Do not email or upload a completed copy through this public website.

District form: Chico Unified School District - BLAST After School Program, 2026-2027 Student & Contact Information, revised August 1, 2026. The fillable convenience copy adds form fields only; district wording and page content are unchanged.

What is not posted publicly

Recurring grade-level schedules, dancer arrival times, backstage or staging locations, pickup arrangements, parent-meeting logistics and family-only calendar feeds are not public web content.

Forms and records

Blank forms may be downloaded publicly. The media release may be electronically signed and emailed directly to RosasdeMaria@RandallStone.net. The CUSD form follows the district's return instructions. Completed Rosas intake, participation and financial-assistance forms must be returned in a sealed envelope and must not be emailed. No completed child or family form may be submitted through a public website upload.

English and Spanish

Family communications are maintained in English and Spanish. Consent, medical, scholarship, privacy and legal forms must use matching controlled versions and receive human review before operational use.
